親友と私の彼が一緒に歩いているのを見かけたので、「何が何だかさっぱりわからない」と言いたいです。

"What on earth!!?" だけで、驚きを表現できます。

同様の言い回しで、"What the heck!!?" と言う表現も頻繁に耳にしますが、こちらは、驚きに加えて不満やネガティブな心境を持つときに、よく使われます。
- What the heck are they doing??!
　あの人たち、何やってんの！？

- What is going on there?
　何が起こってんのかな？

文字通り、何が起こっているかを知りたい時にも使われますが、今回のご質問のように、見たことに驚いて、目を疑うというような状況でも使います。
